Percutaneous Nephrolithotomy

Kidney stones, also known as renal calculi, can be a painful and distressing condition that affects millions of people worldwide. The Surgical Technique
Percutaneous Nephrolithotomy involves the following steps:

  • Patient Positioning: The patient is positioned on their abdomen or side to access the kidney. Imaging techniques such as fluoroscopy or ultrasound are used to guide the surgeon to the precise location of the stone within the kidney.
  • Anesthesia: General anesthesia is administered to ensure that the patient remains comfortable and immobile during the procedure.
  • Insertion of Nephrostomy Tube: A small incision is made in the patient's back, typically in the area of the lower ribcage. A nephrostomy tube is inserted through this incision and guided into the kidney, allowing access to the stone.
  • Stone Removal: A specialized instrument called a nephroscope is then inserted through the nephrostomy tube and into the kidney. The nephroscope contains a light source and camera, which provide the surgeon with a clear view of the stone. The stone is fragmented using a variety of techniques, including laser lithotripsy, ultrasonic or pneumatic devices, or manual extraction tools.
  • Stone Retrieval: The fragmented stone pieces are then removed through the nephroscope or may be flushed out of the kidney using irrigation.
  • Closure: Once the stone has been fully treated and removed, the nephrostomy tube may be left in place for a short period to ensure proper drainage. The incision site is closed, typically with absorbable sutures, and a sterile dressing is applied.

Potential Complications
While PCNL is generally considered safe, there are potential complications to be aware of:

  • Bleeding: Bleeding is a common complication during and after PCNL. In most cases, bleeding is managed during the procedure, but in rare cases, additional interventions may be required.
  • Infection: As with any surgical procedure, there is a risk of infection. This risk is mitigated by the use of sterile techniques and prophylactic antibiotics.
  • Nephrostomy Tube Discomfort: Patients may experience discomfort or irritation from the nephrostomy tube, which is typically temporary and managed with pain medication.
  • Residual Stone Fragments: In some cases, small stone fragments may be left behind, requiring additional treatments or procedures.

Post-Operative Care
After PCNL, patients are closely monitored in the hospital for a brief period. They may receive pain medication and antibiotics as needed. The nephrostomy tube is often left in place temporarily to ensure proper drainage, and it may be removed in a follow-up appointment.
It's essential for patients to drink plenty of fluids to help flush any remaining stone fragments or debris from the urinary tract. Follow-up appointments with the urologist are crucial to monitor progress and address any concerns. Patients should promptly report any signs of infection, excessive bleeding, or worsening urinary symptoms.
